MATRIMONIAL PROCEEDINGS RULES 1988
Rule 2. Form in Schedule
The forms in the Schedule shall be used for the following purposes-
(i) Form 1 shall be used for application for divorce;
(ii) Form 2 shall be used for application for divorce under ta'liq;
(iii) Form 3 shall be used for application for fasakh;
(iv) Form 4 shall be used as Divorce Certificate;
(v) Form 5 shall be Ilsed for application for Certificate of Presumption of Death (Mafqud);
(vi) Form 6 shall be used as Certificate of Presumption of Death (Mafqud).
