After solemnisation and registration have been completed, the bridegroom shall affirm before the two witnesses of the solemnisation and the Registrar the following:

(Name) do solemnly declare that whenever I leave my wife .......................................................................... for a period of four (4) months hijriah or more voluntarily or with force or do not give my wife any maintenance and she makes a complaint to any Syariah Judge and it is found by the Syariah Judge to be true, then she is divorced by one talak.

(2) Whenever I use force or am cruel by abusing or hurting my wife and she complaints to any Syariah Judge and it is found by the syariah Judge to be true, then she is divorced by one Talak.
