ISLAMIC FAMILY LAW (FORMS AND FEES) RULES 1988
Rule 2. Forms in First Schedule
The forms as prescribed in the First Schedule shall be used for the following purposes :
(i) Form 1 shall be used for application for permission to marry for male;
(ii) Form 2 shall be used for application for permission to marry for female;
(iii) Form 3 shall be used as permission to marry under section 15 ;
(iv) Form 4 shall be used as permission to marry under section 16 ;
(v) Form 5 shall be used as Marriage Certificates ;
(vi) Form 6 shall be used as Marriage card ;
(vii) Form 7 shall be used for application for registration of marriage ;
(viii) Form 8 shall be used as Certificate of
