Rule 5.

The forms that are provided in the Schedule for the Work Procedure Mannual are to be used as follows:

5.1. Form JAIPs 1 is to be used for a joint application for divorce without contest.

5.2. Form JAIPs 2 is to be used for application for "Fasakh".

5.3. Form JAlPs 3 is to be used for application for maintenance and other claims for rights.

5.4. Form JAIPs 4 is for application for "Cerai Tebus/Khuluk".

5.5. Form JAIPs 5 is for reports on divorce outside Court and any doubts regarding divorce.

5.6. Form JAIPs 6 is for reports on "Shiqak" (serious disputes between husband and wife).
