ADMINISTRATION OF ISLAMIC FAMILY LAW (FORMS AND FEES) RULES 1987
Rule 2. Forms in First Schedule
The forms as prescribed in the First Schedule to these Rules shall be used for the following purposes:
(1) Form 1 shall be used for registration of particulars of marriages under section 6;
(2) Form 2 shall be used for application for permission to marry for male;
(3) Form 3 shall be used for application for permission to marry for female;
(4) Form 4 shall be used as permission to marry under section 15;
(5) Form 5 shall be used as permission to marry under section 16;
(6) Form 6 shall be used for application for permission to polygamous marriages;
(7) Form 7 shall be used as Marriage Certificate;
(8) Form 8 shall be used for application for registration of marriage under sections 29 and 31.
(9) Form 9 shall be used for application for divorce;
(10) Form 10 shall be used for application for divorce under
ta'liq

(11) Form 11 shall be used for application for
fasakh
;
(12) Form 12 shall be used for application for Certificate of Presumption of Death under section 50;
(13) Form 13 shall be used for application for Certificate of Presumption of Death;
(14) Form 14 shall be used as Divorce Certificate;
(15) Form 15 shall be used as Certificate of
Ruju'
